BHES uses a patient portal to securely send and receive patient forms.

If you need to complete any BHES patient forms, please contact our administrative staff at 815-609-1544, and inform them of the form(s) you
want or need to complete.

Our administrative staff will then will send the form(s) to your BHES patient portal for completion and submission.

If you have not yet set up a BHES patient portal account, please inform
our administrative staff you want to set up an account and they will assist you in doing so.

Once your BHES patient portal account is set up, the requested patient forms will be sent to your patient portal account for completion and submission.
